Remote patient monitoring reduces blood pressure for patients in New York State

Erie County Medical Center, which serves Western New York, implemented a remote patient monitoring program to help manage hypertension in the region, where heart disease fatalities are 33% higher than the national average. The program, in collaboration with vendor Brook Health, includes real-time data tracking and automated alerts. It has resulted in a marked improvement in hypertension control compliance from 44.2% to 74% among patients involved for more than three months.
